1. Introduction

Health Doc Diagnostics (“we,” “us,” or “our”), operated by Dr. Marlo P. Maamo, respects your privacy and is committed to protecting your personal and health information in accordance with the Data Privacy Act of 2012 (Republic Act No. 10173) and its implementing rules and regulations, as enforced by the National Privacy Commission (NPC) of the Philippines.

2. Information We Collect

We collect information you provide directly, including:

• Identity and contact details (name, email address, phone number, address)
• Medical history and intake information (symptoms, allergies, medications, pre-existing conditions) submitted before a consultation
• Appointment, booking, and payment records
• Communications you send us (messages, emails, documents you upload)

3. How We Use Your Information

Your information is used strictly to:

• Schedule and manage your consultations and appointments
• Allow Dr. Maamo to review your history and provide appropriate care
• Generate and issue medical certificates and prescriptions, subject to doctor approval
• Send booking confirmations, reminders, and document delivery by email
• Process payments (GCash or bank transfer)
• Comply with applicable legal, regulatory, and record-keeping obligations

We do not sell, rent, or trade your personal or medical information to third parties for marketing purposes.

4. Legal Basis for Processing

We process your personal and sensitive personal information (including health data) on the basis of your consent, given when you book an appointment or submit an intake form, and where necessary to provide medical services you have requested, in accordance with Sections 12 and 13 of RA 10173.

5. Data Sharing

Your information may be shared only with service providers strictly necessary to operate the platform — such as our email delivery provider (for confirmations and document delivery) and payment processors (GCash) — and only to the extent required to perform those services. These providers are bound to protect your data and may not use it for any other purpose.

6. Data Retention

We retain medical records and booking information for as long as necessary to provide continuity of care and to comply with applicable Philippine medical record-keeping and legal requirements, after which data is securely disposed of.

7. Your Rights

Under RA 10173, you have the right to:

• Be informed that your personal data is being processed
• Access your personal and medical information held by us
• Request correction of inaccurate or outdated information
• Object to or withdraw consent for further processing, subject to legal and medical record-keeping limits
• File a complaint with the National Privacy Commission

8. Data Security

We apply reasonable organizational, physical, and technical safeguards to protect your data, including encrypted connections (HTTPS), access controls limiting medical records to authorized personnel, and secure hosting infrastructure.
