SSS Sickness Benefit has its own eligibility rules, separate from a standard employer medical certificate: the member generally needs to have been confined (at home or in hospital) for at least 4 consecutive days, employed members must notify their employer within 5 calendar days of the sickness or injury, and at least 3 months of SSS contributions must have been paid within the 12 months before the semester of sickness. The certificate documents your diagnosis and confinement period for that claim — the rest of the SSS filing process (notification, contribution records) is between you, your employer, and SSS directly.
